Cognitive restructuring

Help a client notice a thought, examine it, and practice a more balanced response.

4 min readUpdated July 2026

Set the purpose

Use the activity for an agreed pattern that benefits from structured reflection. Explain the steps and practice an example when needed.

Assign clearly

  • Describe the situation or type of thought to notice.
  • Ask the client to record evidence, alternative perspectives, and the effect of the revised thought as appropriate.
  • Choose timing that allows review before the pattern becomes stale.

Review the client’s perspective

Keep in mind

Use client entries as first-person reflections that guide discussion and professional assessment.
